- Tuesday: 8:00 am – 3:45 pm
- Wednesday: 9:00 am – 5:45 pm
- Thursday: 8:00 am – 3:45 pm (No Shots)
- Friday: 9:00 am – 5:45 pm
- Saturday: 9:30 am – 12:45 pm

New Patients
For your convenience, you may print and complete these forms and bring them on your first appointment along with a picture I.D. and the original Insurance card.
All minors must be accompanied by either one of their parents or a letter from their parents allowing the accompanying guardian to make medical decisions on their behalf.
